Detail Changes

If anything has changed in your circumstances just download the appropriate form, fill it in and send it to us at contactus@asteronlife.co.nz or mail it to Asteron Life, PO Box 894, Wellington 6140.

Nominated Beneficiary : This is the form you'll need if you want to add, subtract or change the beneficiaries of your policies.
Policy Owner: Want to change the ownership of your policies? This form is where the process begins.
Name Change: Just married or need to make a change? If your name has been altered in any way, we definitely need to know who we're to pay a benefit to.
Debit and Credit Card: Changed banks or credit cards? This is the form you'll need.
Smoking Update: Have you been smoke-free for over 12 months? Please let us know via this form.
